Challenges in Diagnosis and Management of Second Trimester Omental Pregnancy in Limited Resource Settings: Case Report

Background: Abdominal pregnancies are a rare occurrence and associated with high maternal perinatal mortality rates compared to intra-uterine other ectopic pregnancies. Localization of sites abdominal determining the gestational age at time diagnosis play fundamental role in guiding treatment approach minimizing potential complications. However, vague presentation coupled low accuracy ultrasound detection often leads misdiagnosis pregnancy, hence delaying initiation appropriate management. We present case second trimester pregnancy detected following failure induction for an initial missed abortion, ensuing outcome, rarely reported limited-resource settings. Case presentation: A 19 year old unbooked woman, gravida 2 para 1 17thweek gestation by presented loss foetal movement one week. Based on clinical assessment referral findings, she was initially diagnosed abortion planned induction. Failure prompted repeat which revealed non-viable pregnancy. Laparotomy done, localisation omentum observed dead foetus extracted placenta left in-situ. complication surgical wound dehiscence infection developed post-operatively managed secondary sutures. The patient recovered discharged stable condition. Conclusion: This demonstrates that remains challenge especially settings where skilled human resources health few equipment supplies effective timely limited. sheds some light broader challenges developing countries. Accurate pre-operative requires index suspicion, due variability its presentation. emphasises important quality antenatal care need clinicians conduct comprehensive assessments patients receive training obstetric skills.
